Anaphylaxis

If your child is at risk for an anaphylaxis reaction, please let the office know and complete the following forms:

Other Medical Procedures

If your child is required to take medication at school, you must complete the following form:

All medication must have a prescription label on it with child’s name and dosage. Medications will be secured in the office.
